  1. The intent of the Sponsorship Bonus Program is to reward members who have made a committment to the fitness Lifestyle and who support Spa Lady's continued success by introducing new members to the club. Paramount Acceptance will determine if a sponsorship bonus claim meets the Program's objectives and qualifies for a bonus award. To apply for a sponsorship bonus you must meet the following minimum criteria:
  2. Hold a current LifestyleMembership in good standing at the time of sponsorship,
  3. Be named as the sponsor on the new member's membership agreement, and
  4. Claim your bonus from Paramount Acceptance within 12 months of qualifying and at least 30 days before your membership expires.
  5. To Submit your claim to Paramount Acceptance, follow the instructions on the reverse.
